Your completion of the following form is required in order to receive the free custom analysis service. This custom analysis will be completed for up to 8 hours of work to be completed within 2 months of implementing Ceph. Requesting Ortho2 staff to research the details of particular
measurements for a custom analysis is outside the scope of the free custom
analysis service. All work for those types of custom analysis requests will need
to be billed at our standard rate of $125/hr.
Name: The name of the requested measurement.
Abbr: The abbreviation of the measurement. This may be included on the tracing.
Type: The type of measurement. Please choose one of the following:
- P-P degrees (Plane to Plane Angle)
- P-P (Plane to Plane Distance Ratio)
- P-L mm (Plane to Landmark)
- L-L mm (Landmark to Landmark)
- M-M mm (Measurement to Measurement)

Mean: The optimal value for the measurement.
SD: The standard deviation for the measurement.
Group: The section of the analysis the measurement will be in.
Description: Description
of what we are measuring. When using a plane, include the
two points used to define the plane. Please see the available points list on the
next page.
Reference: If the measurement you are using is used in an existing Ceph
analysis, please note which analysis.
